How Do I Switch to Andrews Pharmacy?

Call or stop by the pharmacy to set up a new patient profile (Personal Info, Phone, Address, Insurance etc.).
Provide a list of medications and name/phone number of current pharmacy for pharmacist to transfer prescriptions.
